Healthy Lifestyle has started two new groups which we hope will help and support our patients.

Lost Friends is held on Thursdays 1.00-2.30p.m here at the surgery. This group is here to support anyone that has suffered a bereavment. It's a small friendly group aimed at helping each other.

The other group is Breathing Buddies. This is also held on Mondays 3.15-4.15 p.m here at the surgery. If you are a C.O.P.D patient, this is for you! Come along and chat with other patients with the same symptoms and get some support and advise.

New Patient Registrationregister_now

Patients can Register between 10:00 and 15:00 Monday to Friday. You will need to attend the Surgery in order to Register. Upon arrival you will be given an application form to complete and then will be offered a simple medical all at the same visit. Please make sure you bring along proof of your Name and Address such as a passport, birth certificate PLUS a household bill or council tax bill.

Medical treatment is available from the date of Registration. Please contact reception for further information.

If your area is not listed above please view our Practice Area Map.  If you live or move to an area not within the area map we can still register you under most cases but please be aware that you will not receive the service of home visits or have access to community services such as District Nurses.  You will be required to sign a consent form to confirm that you understand and agree to the above.
